What will we do?

• Provide robust, impartial assessments of individuals in hospitals, who require ongoing care.
  
• Be a single point of contact for Oxfordshire care providers for updates regarding those clients.
• Support with discharge planning to ensure safe and timely discharges.

What do we aim to achieve?

• Support clients to get home more quickly and more safely, as part of a smoother discharge process, improving the patient experience.
• Better utilisation of care provider time and capacity by removing the need for them to complete a face-to-face assessment, and provide them with a single point of contact based in the acute hospitals.
• Reduce delays in transfers of care, and reduce readmissions following discharge.
