OACP's Trusted Assessor Service provides support to care providers Monday - Friday at the John Radcliffe Hospital to expedite discharge.
We take referrals on:
- local authority funded care home placements
- self-funders seeking a care home placement
- self-funders seeking a home care package
- complex local authority funded home care packages

- 581 assessments completed to 31 October saving the local system £1,472,835
- Plus 1,371 daily updates as requested by care homes

- 2,324 hrs at 4 hrs per assessment

- At a total cost to the Oxfordshire system of just £42,000 to 30/10/2024
